Form Element types

The different types of elements that can be added to a form template

Input Elements are used to request something of the client/prospect, whereas Graphical Elements are generally something for the client/prospect to view.

Please note that Input Elements cannot be placed on the Welcome or Thank You page of a Form Template.

Text field

Single line text field with options to set the minimum and maximum allowed characters.

Textarea field

Multi line text field with option to set the height in rows. Good for requesting longform responses.

Numeric field

Field that only allows numbers with options to set a minimum and maximum allowed value and the option to enable or disable decimal numbers.

Currency field

Field that only allows numeric values that will be automatically formatted as currency. Will add commas to the numbers entered.

Field that allows an option to be selected from a dropdown. You may configure the allowed items for the dropdown. If you want your clients and prospects to have the option of typing a custom value not present in the dropdown, enable the “Allow typing of custom values” option.

Multiple option*

Field that allows the recipient to 'check' off their answer(s). Can enable setting to allow for multiple selections, as well as whether the options are displayed horizontally or vertically.

Phone field

Field to allow the input of phone numbers. It allows the selection of a country code and will automatically format and validate the phone number added.

Email field

Field to allow the input of email addresses. It will validate the format of the email automatically.

Address field

Group of fields to capture all the components of an address. It allows disabling certain parts of the address such as address lines, city, postal code, state, and country. Can enable/disable USPS Address Validation.

Percentage field

Field to allow the input of a percentage value with options to set the minimum and maximum allowed value as well as an option to enable or disable decimal values.

Date field

Field to capture a date. It shows a calendar when clicked and will automatically validate and format the selected date.

Year field

Field to capture a year. It shows a calendar when clicked and will automatically validate the year selected.

Calculated fields do not allow the user to input a value. Instead, they perform a series of calculations using values from other elements. You can select to hide the calculated field by setting its visibility to hidden. Calculated fields will have one of three types: text, numeric and currency. Depending on the type, the operations available are different. Text calculated fields will allow you to join text elements together as well as calculate an age based on a date field. Numeric and currency fields will allow you to sum, subtract, multiply and divide numeric values. You can nest multiple operations together in the calculations editor.

Encrypted field

Field to capture sensitive information such as social security numbers. It allows selecting the minimum and maximum allowed characters as well as the amount of characters to display unencrypted. When a value is added to this field, it will automatically be encrypted in the system. The user’s password will be required to decrypt the value and see it in full. 

Field to allow the upload of documents. Can accept the following file types:

Image upload

Field to capture images/photos. It allows the upload of an image file and will show that image resized to fit the current size of the element.

Please note: If you would like the images you gather to be bundled with the documents that have also been uploaded to an engagement, you may want to use a Document Upload Element for everything, as these accept image files, as well.

Electronic signatures are available in templates with high security level only. An e-signature element will allow the client or prospect to draw or type their signature. Each e-signature element may apply to the entire template, only to the page where it is added or only to the group where it is added. Depending on this option, the generated signed document will include the content to which the signature applies. Your clients and prospects will have the ability to download the signed document after they complete the engagement. 

Textbox (graphical element)

Element that allows the input of text. The content editor for this element allows simple formatting such has headings, sub-headings, text alignment, bold, italic, lists, etc.

Image (graphical element)

Element to upload and display an image. This element will only display the image and does not allow uploads by the client or prospect.

Document download (graphical element)

Element to allow the download of a document by the client or prospect. 

Logo / Photo (graphical element)

Element to display your firm’s logo or your user photo. Pulls Logo from Teams domain and pulls Photo from the user's PreciseFP profile photo

Video (graphical element)

Element to embed a video. You can embed one of our default videos or paste a link to a video from Youtube, Wistia, Vimeo or any video available over standard HTML5 video players.

Button (graphical element)

Element to display a button that opens an external link. You will be able to configure the color and label of the button as well as whether the external link is a URL or a 'Mailto' option the client/prospect can use to contact an email address of your choosing.

Badge (graphical element)

Element to display a value in a badge. You will be able to configure the shape and color of the badge and select the source element that contains the original value to display. This element works very well in conjunction with calculated fields to show scores or grades calculated based on a series of answers from your client or prospect.

Calendar App (graphical element)

Element to embed your calendar within the template. The Calendar App element supports integrations with Calendly, Acuity Scheduling, Schedule Once and ZynCal. Does not open a new tab/window for the client/prospect when scheduling.

Element to display the advisor's signature. Available inputs to add your signature are typing, drawing, or you can upload an image of your signature.
